Consumers can now upload a copy of advance care planning documents such as their advance care directive or power of attorney directly to their My Health Record (MyHR).

As revealed by Pulse+IT in February, the system has been reconfigured to allow consumers to upload the documents as a PDF, where it can be viewed by healthcare providers or appointed representatives.
